All applications must be submitted to the Program Office no later than 5:00 p.m. January 31, 2010.  All Federal applications forms and supporting documentation must be complete before the application will be accepted.

IS-800b

 

First Responder Operations Level for
Hazardous Materials (OSHA 29 CFR, 1910.120). Technician level is also valid.

 

Current CPR card

 

Current EMT/Paramedic (paramedics please provide copies of PEP/PALS/ACLS if available)

 

Valid Utah drivers license (Class "B" or "A" may become Task Force drivers, "A" drivers must provide current medical examiners certificate)

 

Firefighter I or II
